Ashwagandha, often called Indian ginseng or winter cherry, is one of the most researched adaptogenic herbs in modern science. Used for centuries in Ayurvedic medicine to promote vitality and resilience, it has now been examined in dozens of randomized controlled trials (RCTs) and multiple meta-analyses. Its primary active compounds—withanolides—appear to help the body manage stress by modulating the hypothalamic-pituitary-adrenal (HPA) axis, supporting neurotransmitter balance, and reducing inflammation.

Here's a clear look at the strongest evidence for its benefits.

1. Stress, Anxiety & Cortisol Reduction (Strongest Evidence)

This is where the research is most consistent and robust. Multiple systematic reviews and meta-analyses of RCTs show that ashwagandha significantly lowers perceived stress, anxiety scores, and serum cortisol levels compared to placebo.

  • A 2026 dose-response meta-analysis of 22 RCTs (1,391 adults) found large reductions in stress, anxiety, and depression symptoms. Benefits were stronger at lower doses (≤500 mg/day) and longer durations (>8 weeks).

  • A 2024 meta-analysis of 9 RCTs (558 people) reported meaningful drops in Perceived Stress Scale scores, Hamilton Anxiety Scale scores, and cortisol.

  • Classic trials (e.g., Chandrasekhar 2012 using 600 mg/day KSM-66 extract) showed ~28% reductions in cortisol and large improvements in stress and anxiety scales.

It appears to work by calming overactive HPA-axis signaling rather than simply sedating the system.

2. Sleep Quality

Evidence is moderate but positive, especially for people with insomnia or high stress. A meta-analysis of five RCTs found improvements in sleep onset latency, total sleep time, sleep efficiency, and overall sleep quality. Benefits were clearer at doses around 600 mg/day for at least 8 weeks.

One well-known 2020 trial (150 participants) using 300 mg twice daily showed faster sleep onset, longer sleep duration, and better efficiency versus placebo. People often report feeling more rested without next-day grogginess.

3. Cognitive Function

A recent systematic review and meta-analysis (20 studies, 1,249 participants) found that ashwagandha improved memory, attention/processing speed, and executive function. Effects on visuospatial ability were also positive after accounting for one outlier.
